Health Problems/Concerns
Please notify the school nurse, with any health problem (asthma, seizures, diabetes etc.). The school nurse may ask you for further information. You may be asked to complete specific healthcare plans. It is very important to return this information in a timely fashion. Your health provider's signature may be required on these care plans. Keeping your school nurse informed and your child's healthcare needs updated will provide a healthful learning atmosphere.
Action Plans
For students who suffer from a chronic condition or potentially life-threatening allergy, a parent must sign an Action Plan completed by the student's physician and submit it to the school nurse.

Registration Forms
To be completed upon registration:
- Student Annual Health Information Form — this includes CHIRP consent and consent to treat students in case of emergency.
- Physical Forms (required for kindergarten and grade 6 students and those participating in school-sponsored athletic programs)
- Kindergarten through grade 5 students should complete the Tri-Creek Child Health Examination form.

Grade 6 through grade 12 students should complete the IHSAA Pre-participation Physical Evaluation Form.
